· The complete best practice can be found in the file called AMP – Apprentice Mentoring Program. It includes the main document, as well as the five appendices, all in PDF format.
· Appendix A includes sample forms and templates such as mentee and mentor application forms, a mentoring agreement and a mentoring action plan. Each of these files is presented in Word format so the information can be tailored to meet the needs of your program.
· Appendix B consists of implementation tools that can be helpful when putting an AMP in place.
· Appendix C includes various job descriptions that relate to an AMP. These are also presented in Word format so you can modify them to meet your company’s needs.
· Appendix E includes evaluation templates that can be used to help measure the success of your AMP. Once again, these are offered in Word format so they can be customized to meet the needs of your specific program.
